What to complete

Your contact details, health history, and the basics we need to care for you safely.
If you have dental insurance, add your details so we can file your claims as a courtesy.
Prefer to fill them out in person? No problem. Just arrive about fifteen minutes early and we will have everything ready for you.

A valid photo ID
Your insurance card, if you have one
A list of any medications you take
Your pharmacy and primary care doctor’s information, if you have it

The information you share is kept private and secure, and used only to provide your care. We follow all applicable privacy standards, including HIPAA.
